Physician information

• Hospital affiliations
  Information includes a listing of hospitals where each provider has medical privileges, plus the type and status of privileges, and effective dates. Past or present restrictions also may be documented.
  
  
• Professional experience
  Document your providers’ professional experience with dates, practice name, address, country, zip code, phone number, title, department and beginning and ending dates of employment. CETrac will automatically sort the data based on the beginning work date.
  
  
• Managed care plans
  Import care plans from the Managed Care Plan files to display the provider’s participation status. In addition to information stored in the billing system, the Care Plan ID and type of affiliation, such as PPO, HMO, etc., are identified. This data is required for many standardized state forms.
  
  
• Liability insurance
  Store each provider’s liability insurance carrier history by date.
  
  
• Suits/settlements
  Document detailed information on actions opened, closed or pending by provider.
  
  
• CME tracking
  Track the number of CME hours needed to maintain certification and licensing in each state in which the provider is licensed. Enter each continuing education event with descriptions, dates, hours by category and any comments. Then generate a report showing additional requirements needed to maintain current licensing or certification.
  
  
• Business investments
  Enter investment and/or business interest information for clinical laboratory and diagnostic centers by provider.
  
  
• Provider questionnaire
  Once answers are entered in this questionnaire, the information can be included in forms provided by CETrac or those you customize.
  
  

  CETrac provides two forms:
  • Provider Application Form, which can be used to apply for participation in a care plan
  • Standardized Credentialing Form

  You may add, change or delete questions to customize forms to meet your own state requirements on nurse and provider application forms, state credentialing forms and more.
